  • SCHEDULE CHANGE FORM - DIRECTIONS:  Please complete this form if you find a mistake on your student schedule and return it to the CHS Counseling (Guidance) Office.  To assist students as quickly as possible, appointments will not be made with counselors to adjust student schedules prior to the start of school.  If your request cannot be accommodated or more information is needed, you will be contacted by your child's counselor.
  • LEVEL CHANGE FORM - Directions: Please complete the Level Change Request Form if you are requesting to change levels in the same course.  All necessary signatures are required before this form can be submitted to a Department Director/Chair for approval.  {This form must be completed for a level change up or down}
    The file link above provides the Request Form for a Letter of Recommendation as well as directions for requesting a teacher letter of recommendation through your Naviance Student Account. All students requesting a letter of recommendation from a teacher, must first complete the attached form and hand deliver it to the teacher you are seeking the letter of recommendation from. The next step is to electronically request the letter of recommendation through Naviance Student. If you have any questions please see your guidance counselor.

  • Transcript Request Form for Connetquot Graduates
    This Transcript Request Form is for Connetquot High School Graduates. Please complete the form and return it by fax, email or hard copy to the HS Registrar.  If you have any questions, please contact the HS Registrar's Office at (631) 244-2228, ext. 1096.

  • CollegeBoard - Consent Form for Accomodations Request
    If you would like to apply for testing accommodation(s) on College Board tests (SAT, PSAT/NMSQT, and/or Advanced Placement Exams) due to a documented disability, please click on the link above for a copy of the “CONSENT FORM FOR ACCOMMODATIONS REQUEST”. Complete the form and mail it to our Services for Students with Disabilities Coordinator and Director of Guidance, Mr. Jonathan Macaluso. The signed “CONSENT FORM” will authorize the high school to release to the College Board copies of your records that document the existence of a disability and need for testing accommodations; to release any other information in the school's custody that the College Board requests for the purpose of determining your eligibility for testing accommodations on College Board tests; and to discuss your disability and accommodation needs with the College Board. 

  • FOR SENIORS ONLY - COLLEGE/UNIVERSITY TRANSCRIPT REQUEST FORM  - Transcript Request Form for College Applications - Connetquot High School (This form is utilized by seniors for the college application process. After completing this form students must return this form to their guidance counselor.

  • Working Papers - Directions
    This pdf file above provides directions for obtaining working papers. After reading the directions you will need to complete the following forms: 1) AT-17 Application for Employment Certificate 2) AT-16 Physical Fitness Certificate

    Follow the directions carefully and complete Part I and Part II of this form.

    Fill in the top part of the form. Parts A and B must be completed by the school physician or your own doctor at your expense.
