  • SCHEDULE CHANGE FORM - Directions: Please return the completed form to the High School Guidance & Counseling office by mail, email to chsguidance@ccsdli.org, or drop it off to the HS Main Lobby.  Appointments will not be made with the counselors to adjust student schedules.  If your request cannot be accommodated or more information is needed, you will be contacted. 
    *Please keep in mind that course requests made last school year are final.  Teacher requests, period changes, or lunch changes will not be accommodated.
    **If you are requesting a level change up or down please note that the Level Change Form (see next bullet) must be completed.  No changes can be made without prior approval.

 

  • LEVEL CHANGE FORM - Directions: Please complete the Level Change Request Form if you are requesting to change levels in the same course.  All necessary signatures are required before this form can be submitted to a Department Director/Chair for approval.  
    (This form must be completed for a level change up or down)

 

  • TEACHER RECOMMENDATION FORM - Directions: All students requesting a letter of recommendation from a teacher must first complete the attached form and hand deliver it to the teacher you are seeking the letter of recommendation from. The next step is to electronically request the letter of recommendation through Naviance Student. If you have any questions please see your guidance counselor.

 

  • Transcript Request Form - Directions: This Transcript Request Form is for Connetquot High School Graduates ONLY. Please complete the form and return it by fax, email or hard copy to the HS Registrar.  If you have any questions, please contact the HS Registrar's Office at (631) 244-2228, ext. 1096.

 

  • CollegeBoard - Consent Form for Accomodations Request
    If you would like to apply for testing accommodation(s) on College Board tests (SAT, PSAT/NMSQT, and/or Advanced Placement Exams) due to a documented disability, please click on the link above for a copy of the “CONSENT FORM FOR ACCOMMODATIONS REQUEST”. Complete the form and mail it to our Services for Students with Disabilities Coordinator and Director of Guidance. The signed “CONSENT FORM” will authorize the high school to release to the College Board copies of your records that document the existence of a disability and need for testing accommodations; to release any other information in the school's custody that the College Board requests for the purpose of determining your eligibility for testing accommodations on College Board tests; and to discuss your disability and accommodation needs with the College Board.
